For GPs and referrers
Your patient, and you kept in the loop.
We work alongside the GP, not instead of them. Here's how to refer and what you'll hear back.
What you'll receive
A letter after every appointment.
With the patient's consent, the treating doctor writes to you the same day with what was discussed and decided.
- Assessment and plan
History, records reviewed, options discussed (including none), the plan and the reasons for it.
- Any prescribing
What was prescribed, the dose, the review date and what to watch for, so your records are complete.
- Side effects and changes
Anything reported, what we did, and what we reported to the TGA.

How to refer
Send a referral the usual way.
Send your referral letter by secure messaging to calder-health [placeholder]. Use this form only to tell us it's on its way, or to ask a Calder doctor to call you.
No clinical details here, please. This web form isn't for health information; send that by secure messaging.
